ALAMO DIABETES TEAM'S DIABETES
SELF-MANAGEMENT TRAINING
The National Standards for Diabetes Self-Management Education requires that programs track their participant's outcomes related to diabetes self-management.
Alamo Diabetes Team tracks the following outcomes:
• Achievement of diabetes self-care goals
• Blood sugar control. (A1C is a blood test that measures the
average level
of diabetes control over the previous
} 24% reduction
in A1C level
2 - 3 month time period) At intervals the diabetes educators request the latest A1C test results from the primary care provider's office.
• Trends in lipid profile; this includes total cholesterol, HDL,
LDL cholesterol and triglycerides (or blood fat).
• Use of healthcare services such as hospital admissions and visits to the emergency room for diabetes care.
