Using the correct forms to capture all the required data elements on your participants is a critical part of any diabetes education program.  The time investment in developing these forms, pilot-testing them, and making revisions is significant, and can negatively impact your staff’s productivity. 

Using the Diabetes Self-Management Program Forms Package developed and tested by Alamo Diabetes Team, LLP can save your new business considerable start-up time and staff resources.   Our copyrighted forms include all the data fields needed to comply with the documentation standards for diabetes education programs. 

Under this licensing agreement, your business’s name and logo is embedded into the program forms you select.  Alamo Diabetes Team, LLP then sends you a print-only CD-ROM version of the forms package, which you can use to print forms as needed at your facility.
